မိဘ/ဂါးဒီးယန်းတစ်ဦးသည် Familyid.com တွင် မှတ်ပုံတင်နိုင်သည် သို့မဟုတ် သင့်ကလေး၏ ကျောင်း ဝက်ဘ်ဆိုက်တွင် လင့်ခ်ကိုနှိပ်နိုင်သည်။ လမ်းညွှန်ချက်များကို အောက်တွင်ဖော်ပြထားပါသည်-

- အကောင့်ပိုင်ရှင်ဦးနှင့် နောက်ဆုံးအမည်များ (မိဘ/ဂါးဒီးယန်း)၊ အီးမေးလ်လိပ်စာနှင့် စကားဝှက်ကို ဝင်ရောက်ခြင်းဖြင့် သင့်စိတ်ချရသော HomeID အကောင့်အတွက် လက်မှတ်ရေးထိုးပါ။ ဝန်ဆောင်မှုလုပ်ငန်း၏ သဘောတူညီချက်ကို ရွေးချယ်ပါ။ ဆိုင်းဘုတ်ကိုနှိပ်ပါ။
- သင်၏ အကောင့်အသစ်ကို လှုပ်ရှားရန် လင့်ခ်တစ်ခုဖြင့် အီးမေးလ်တစ်စောင် ရရှိပါလိမ့်မည်။ အီးမေးလ်ကို မတွေ့ဘူးဆိုရင် အီးမေးလ် စစ်ထုတ်ကိရိယာတွေကို စစ်ဆေးပါ (စာမေးလ် ဖိုင်တွဲ)ကို စစ်ဆေးပါ
- လှုပ်ရှားမှု အီးမေးလ်ထဲက လင့်ခ်ကိုနှိပ်ပါ။ အဲဒါက သင့်ကို ဝင်ခွင့်ပေးပါလိမ့်မယ် FamilyID.com
- မှတ်ပုံတင်ရေးပုံစံအတိုင်း တောင်းဆိုထားသော သတင်းအချက်အလက်ကို အပြီးသတ်လုပ်ဆောင်ပါ။ အနီရောင်ရှိတဲ့ လယ်ကွင်းအားလုံး အဖြေရဖို့လိုတယ်။
- သင်၏ပုံစံ ပြည့်စုံလာသောအခါ သိမ်းဆည်းပြီးဆက်သွယ်ရေး ခလုတ်ကိုနှိပ်ပါ။
- မှတ်ပုံတင်ခြင်း အကျဉ်းချုပ်ကို ပြန်သုံးသပ်ပါ။
- စိမ်းလန်းတဲ့ Submit ခလုတ်ကိုနှိပ်ပါ။ 'Submit' ကိုရွေးချယ်ပြီးနောက် မှတ်ပုံတင်ခြင်း ပြီးဆုံးသွားပါလိမ့်မယ်။ သင် ၏ မှတ်ပုံတင် ခြင်း ကို အတည်ပြု သော ReID မှ အပြီးသတ် အီးမေးလ် တစ် စောင် ကို သင် လက်ခံ ရရှိ လိမ့်မည် ။
You will receive an email receipt from registrations@familyid.com. You can also view your completed registration in your 'Registration' tab. You can log into https://www.familyid.com at any time to update your information and to check your registration(s).

ONLY Parents/Guardians Can Sign Off Electronically on the Agreements
Note: If a Student Athlete electronically signs off on these agreements, he/she will NOT be cleared and could prevent them from starting on time.
